NOTICE:  By signing this form, you agree to relinquish all rights and interest in this animal to HART, and you release HART and its members and volunteers from all liability arising from the surrender, vet care, transport, foster, adoption or transfer of this animal.  Your signature also affirms that all statements are true to the best of your knowledge. You authorize permission for the release of all relevant veterinary records to any authorized HART representative.

HART may foster and/or adopt the animal out to another home locally, or transfer the animal to a no-kill affiliate rescue out of state.
